A new report cited pilot error for a crash that killed a well-known businessman and others. The National Transportation Safety Board said that a flight that contained Lewis Katz crashed in part because the pilots failed to perform routine preflight checks and there was an issue with the gust lock system before take off and the pilots didn’t do anything about it.
