Some of the early details have been released in the investigation into a plane crash that led to the death of Philadelphia Inquirer owner Lewis Katz. According to the National Transportation Safety Board investigation, only twice in the last 176 did the pilots do a full safety check of the plane before take off. The NTSB hasn’t assessed blame yet, but repeatedly hit on the lack of safety measures that the two pilots took on flights dating back.
